Please be aware that older browsers have communication issues when working with an advancing
Internet. We suggest you make sure you have a recent version of your favorite web
browser. You can check the version of the browser you are using by clicking the word
"Help" on your browser's text
toolbar. Under the Help menu, click on "About..." and it should tell you what version
you have. If you have Microsoft Internet Explorer 3.0 or earlier, Netscape 3.9 or
earlier, or AOL 4.0 or earlier, or just want to be sure you have the latest version of your
browser, please upgrade your software.

Please navigate the survey using only the buttons provided at the bottom
of each page. Using other navigation methods, such as a "back" button on a mouse,
may result in errors which can prohibit you from finishing the questionnaire. For
this reason, this browser window opened without the normal toolbars and
buttons.
Answer each question by selecting the response category or categories that
best describe your opinion, or by clicking and filling in the response box.
If the page is longer than your viewable screen, scroll down the screen to
see the remaining responses or questions. When you have answered all the
questions on a page, click the "Next" button at the bottom of the page to
move to the next page.
You may move backward by using the "Back" button at the bottom of the screen; if
you move back more than 1 question, you may have to re-fill the intermediate
screens with your responses.
If you should have to stop while taking the survey, click on the
"Suspend" button, and your answers will be preserved so that you may
return and complete the survey at a more convenient time.
Please be sure to note the URL as well as your user name (if applicable) and password.
You will need them to resume your survey.

You should be able to use the "PREVIOUS" button at the bottom of
the survey page to return to previous questions. If you have to back up more
than a few questions to correct a mistake, you may have to re-enter data on
subsequent questions.

At times the "PREVIOUS" button will be removed and you will be
unable to return to a previous question to correct your answer. We do this in
order to protect a specific methodology the client wishes to use. In these
cases, please fully complete the questionnaire. Then send us an email which
includes your username (if applicable), password, the link you used to access
the survey and the information you feel should be amended to
SurveySupport@catalystmr.com.
- Sometimes you may not realize a mistake until you have completed the survey. In this case, please send us an email at
SurveySupport@catalystmr.com
which includes the survey information from your invitation. It is particularly
important that you include your password and, if applicable, your username, as well
as the link you used to access the survey. Please let us know what information you
feel you entered incorrectly and we'll be happy to make the correction for you.

To finish a partially completed survey, simply return to the url, or web address, provided
in your invitation. Re-enter your user information as requested by the opening
page. The survey will remember where you left off and will allow you to finish
the survey from that point.

Are you certain the URL or web address is correct? Please check
for typos in the URL entered in your browser's address bar. Be sure
capitalization is correct since web addresses are often case sensitive.

Occasionally issues we cannot control may prevent respondents from
reaching a survey. These issues, which can include Internet congestion, connectivity
problems with your Internet Service Provider , or even catastrophic weather, are almost always temporary. Most users will be able to access the survey by simply trying again a bit later.
- If you have continued difficulties accessing the survey, send an email to
SurveySupport@catalystmr.com. In your email, we ask that
you be as specific as possible about the problems you are experiencing. Please let us
know what web address you used to reach our site, how you learned about the survey, and
what Internet browser you're using. If possible, please include the browser's version number.
We will resolve your situation as quickly as possible.

- Dropped calls and Internet Service Provider congestion can sometimes cause your service to be interrupted.
Be sure you are still connected to the Internet and try the link again.
- Some older email programs have trouble with URLs if they are broken by a line wrap.
For instance, if the link wraps to the next line and only the first part of the URL is underlined,
you might have a problem reaching our website. Try highlighting the entire link, copying
it and then pasting it into your browser's address bar.
- If you still cannot reach the survey, please send us an email to

including the survey link, password, and (if applicable) username you were
provided.

- Passwords are assigned individually for our surveys. If you look in the invitation
which brought you to our website you should find the password assigned to you. If you
no longer have the invitation, you can email us at
SurveySupport@catalystmr.com
and we will try to help you. Please be aware that there will be times when we will not be able to
supply passwords (for instance if a study has concluded).
- Very often problems with passwords result from typos or incorrect capitalization. Passwords
must be typed exactly as assigned. Click on the password entry box and
use the "Backspace" button on your keyboard to be sure you're on the farthest left part of the
box. If you received your invitation as an email, highlight and copy the password, then
paste it into the password entry box. If not, type in your password exactly as it
appears on your invitation. If this still doesn't work, please send us an email to
SurveySupport@catalystmr.com.

Error Code 9122 - This error may have been caused by double clicking on the start button.
By returning to the survey URL and entering your password and (if applicable) username you should
be able to begin the survey.
- "Fatal Error request method Get not post" or error in line 114/115. -
These error messages are seen by AOL or WebTV users using older versions of their service's
browsers.

AOL users - If you are using AOL 4.0 then you need to upgrade
your software. Do this by using AOL keyword "upgrade" and following the
instructions provided. If you are using AOL 5.0 and a get a message
reporting an error on line 114 or 115 then you can just click "OK" in the dialogue
box and you will be able to continue the survey with no problem.
Alternatively, a possible solution is to login to AOL and then use a different
browser like Internet Explorer or Netscape to access the survey.
WebTV users - We have noticed that WebTV customers are having a problem.
WebTV's browser does not always interpret javascripts properly. If you
can access the Internet from another browser, you should be able to take the
survey.

- Sequence Error - This error can occasionally be caused by clicking the navigation
buttons (ie "PREVIOUS" or "NEXT") more than once. It can also be caused if you use your
browser's "BACK" button rather than the survey's "PREVIOUS" button. Generally by using only
the survey navigation buttons and avoiding any other browser controls you should be able to
continue with the questionnaire. If you are unable to finish a survey because of this, please
email us the URL you used to access the study, your password and (if applicable) username to
SurveySupport@catalystmr.com.
